Medical Condition Support

When requesting membership for each club or society you wish to join you will be asked to answer YES or NO to the Health & Safety question as shown below on this page.

If you select YES, you will be prompted with the following:

You have clicked YES to having a medical condition that is likely to affect your ability to participate in the activities of the {CLUB/SOCIETY NAME}!

You are required to provide a letter from your family GP which states that you are fit to take part in the activities of the {CLUB/SOCIETY NAME}. The letter must be presented to the Clubs and Societies Office before you can partake in any activities.

Once you submit a membership request with YES selected, you should be emailed the following message, with an attached template to bring to your GP:

Hi {FIRST NAME},

You indicated that you may have a medical condition that prevents you from taking part in the activities of the {CLUB/SOCIETY NAME}. This requires you to present a letter from your family GP to the Clubs and Societies Office stating that you are fit to take part in the activities of the {CLUB/SOCIETY} before you can partake in any activities.

If you did not receive this email, please click here to download the template medical letter to bring to your GP.

THIS LETTER MUST BE BROUGHT TO THE C&S OFFICE STAFF, NOT THE CLUB/SOCIETY COMMITTEE.

Once the C&S Office has receiced this, they will enter it into the system and you will then be able to active your membership by meeting a committee member.

Please note: If in doubt, you must disclose anything which you think might be relevant. For example, if you have any cardiovascular or respiratory condition (such as asthma or high blood pressure) and if club activities require vigorous physical exercise, then this should be disclosed.

Please also note that UL Wolves clubs and societies act on an inclusive basis and disclosure of any illness or condition usually does not led to membership being declined and it is only in very unusual circumstances that membership is declined altogether. However, a doctor's certificate will be required so the necessary safeguards can be taken for the safety of all.
